CHILDREN TUBERCULOSIS IN THE FAMILY DOCTOR'S PRACTICE Algirdas Andriuska, Dalia Steponaviciene Kaunas 2nd Clinical Hospital, Kaunas, Lithuania
Aim of the study: to evaluate the possibilities of control of the children tuberculosis (TB) in Primary Health Care Centers (PHCC). Study object and methods: the family doctors (FD) working in PHCC. 199 questionnaires answered by FD were analyzed. The statistical analysis was done with SPSS software for data acquisition and analysis. Results: Only 64,3% of FD in their PHCC had the possibility to make tuberculin skin test (TST) and X-rays of the chest. Only 26,1% of FD got the information about TB positive persons and children of risk groups from TB hospitals and outpatient departments. Only 47,2% of FD knew about TB positive persons and children of risk groups in their area. Half of the medical personnel in the cities and quarter of the personnel living in countryside took part in the refresher courses about TB control and prophylaxis. The 73% of FD think that DOTS (Directly Observed Treatment, Short Course Chemotherapy) must be given in specialists’ – pulmonologists’ offices, but not in the PHCC. In the article we present three cases of so late diagnosed children TB. Conclusions: Only 64,3% of FD in their PHCC had the possibility to make tuberculin skin test and X-rays of the chest (due to the lack of finances, work conditions and public unconcerned about it’s health). To implement the National program for TB control and prophylaxis the consistent refresher courses devoted to the control of TB should be organized for the medical personnel; PHCC should be provided by means for TB prophylaxis, diagnostic and control; the public teaching about TB prophylaxis should be more active.
